Boarding Admission

Waiver

I have previously filled out this form and to the best of my knowledge nothing has changed since that time. I understand that except for spa services, my pets will be treated based on the previously filled out boarding form. If spa services are requested again, please note on the form. (Please leave form blank and sign at the bottom of the form.)

Boarding Policies

  • All boarding guests must have a current examination, vaccinations and be parasite free. Towne Park Animal Care Center will provide these services at an additional charge if documentation is not provided at check-in.
  • Canine Vaccine Requirements: Rabies, Distemper Combination, Bordetella, and Bivalent Canine Influenza.
  • Feline Vaccine Requirements: Rabies and Distemper Combination.
  • If your pet will be picked up by someone other than yourself, please inform our staff at check-in.
  • Towne Park Animal Care Center does not assume liability for any lost or damaged belongings.
  • All reasonable precautions will be used to prevent any injury or escape of your pet.
  • Towne Park Animal Care Center is not responsible for the actions of the pet that may cause injury or escape.
  • Any pet that is found to have fleas on check-in will be given a single dose of flea prevention and added to the bill for their stay.

Spa Services

All spa services are performed the day before discharge unless otherwise requested. Guests staying seven nights or longer will receive a complimentary bath.

Medical Care

  • If a guest becomes ill while boarding, Towne Park Animal Care Center will attempt to contact the owner or the provided emergency contact immediately.
  • In the event the owner or emergency contact cannot be reached, and the pet requires emergency treatment, Towne Park Animal Care Center will treat the pet and the owner will assume full financial responsibility under the guidelines authorized below. Please choose only one option.
